Name, age, phone number, gender, weight, height, pulse, drug history, OFC, RR, hip, waist, address, occupation, education, and email.

Medical Information:

Consultation fee, blood pressure, temperature, marital status, chief complaint, examination findings, diagnosis, and related medical records.

Images:

Patient disease images, investigation/report images, and doctor profile images.

2. Purpose of Data Collection

We collect and process data to:

• Create and maintain accurate patient profiles
• Support diagnosis and treatment
• Monitor health conditions
• Generate and manage medical prescriptions

All personal and medical data are securely stored in a protected database. Access is strictly limited to authorized doctors and medical staff. We implement appropriate technical and organizational security measures to prevent unauthorized access, disclosure, or misuse.

রোগীর তথ্য:

নাম, বয়স, ফোন নম্বর, লিঙ্গ, ওজন, উচ্চতা, পালস, ওষুধের ইতিহাস, OFC, RR, হিপ, কোমর, ঠিকানা, পেশা, শিক্ষা এবং ইমেইল।

চিকিৎসা সংক্রান্ত তথ্য:

পরামর্শ ফি, রক্তচাপ, তাপমাত্রা, বৈবাহিক অবস্থা, প্রধান উপসর্গ, শারীরিক পরীক্ষার ফলাফল, রোগ নির্ণয় এবং সংশ্লিষ্ট চিকিৎসা নথি।

ছবি:

রোগীর রোগের ছবি, পরীক্ষা/রিপোর্টের ছবি এবং ডাক্তারের প্রোফাইল ছবি।

২. তথ্য সংগ্রহের উদ্দেশ্য

সংগৃহীত তথ্য ব্যবহার করা হয়নি ম্নলিখিত উদ্দেশ্যে:

• সঠিক রোগীর প্রোফাইল তৈরি ও সংরক্ষণ
• রোগ নির্ণয় ও চিকিৎসায় সহায়তা
• স্বাস্থ্য পরিস্থিতি পর্যবেক্ষণ
• চিকিৎসা প্রেসক্রিপশন প্রস্তুত ও পরিচালনা

৩. তথ্য সুরক্ষা
